Indications:
  • Treatment of Cryptococcal Meningitis in HIV infected patients.
  • Treatment of patients with Aspergillus species, Candida species and/or Cryptococcus species infections refractory to amphotericin B deoxycholate.
  • In patients where renal impairment or unacceptable toxicity precludes the use of amphotericin B deoxycholate.
  • Treatment of visceral leishmaniasis, in immuno-compromised/ immunocompetent patients.
  • Empirical therapy in febrile immunocompromised patients who do not respond to broad-spectrum antibiotics.

Dosage:
  • For systemic mycoses
  • For prophylaxis against invasive fungal infections in liver transplant recipients
  • In HIV-associated disseminated cryptococcosis
  • Visceral leishmaniasis (immunocompromised patients)
  • For empirical treatment of presumed fungal infection of febrile neutropaenic patients
  • 1mg/kg/day of body weight, and increased stepwise to 5.0mg/kg/day, as required.
  • 1mg/kg/day for five successive days following transplantation
  • 3mg/kg/day for up to 42 days
  • 1 to 1.5mg/kg/day for 21 days may be used
  • 1-3 mg/kg. The daily dosage may be adjusted based on the clinical condition, as required.

Indications:

It is indicated in the treatment of:
  • Systemic mycotic infections due to organisms susceptible to Amphotericin B,where toxicity precludes the use of conventional systemic Amphotericin B therapy. Infections such as disseminated candidiasis, mucormycosis, aspergillosis, cryptococcosis, histoplasmosis have been successfully treated with Liposomal Amphotericin B.
  • Fever of unknown origin in neutropenic patients where the fever has failed to respond to broad spectrum antibiotic therapy and appropriate investigations carried out have failed to establish the cause as bacterial or viral.
  • Visceral leishmaniasis in both adults and children.

Dosage:
  • Adults: Not necessary with renal impairment if decreased renal function is due to amphotericin B the daily dose can be adjusted.
  • Children: 5mg/kg given as a single IV infusion daily.

Dosage Adjustment:
  • Renal Impairment: Not necessary with renal impairment if the decreased renal function is due to amphotericin B the daily dose can be adjusted.
  • Hepatic Impairment: Administer with caution.

Indications:
  • Presumed fungal infections in febrile neutropenia, as empiric therapy. Candida infections of the esophagus, blood, peritoneum, intraabdominal abscess, pleural space. Invasive aspergillosis in patients refractory to or intolerant of other therapies (not for initial therapy).

Dosage:
Adult:
  • Give by slow IV infusion over 1hr. ¿¿¿18yrs: Esophageal candidiasis: 50mg once daily. Other Candida infections, aspergillosis, or empiric: 70mg once on Day 1 then 50mg daily. Empiric: treat for at least 14 days; continue for at least 7 more days after resolution of neutropenia and clinical symptoms; may increase to 70mg/day. All others: continue at least 14 days after last positive culture. Moderate hepatic impairment: 35mg daily (may give 70mg on Day 1 when indicated). Concomitant rifampin: 70mg once daily. Concomitant other CYP inducers (eg, nevirapine, efavirenz, carbamazepine, dexamethasone, phenytoin): consider 70mg once daily.

Children:
  • <3months: not established. Give by slow IV infusion over 1hr. 3mos-17yrs: 70mg/m2 once on Day 1, then 50mg/m2 daily. May increase to 70mg/m2 daily; max 70mg. Empiric: treat for at least 14 days; continue for at least 7 more days after resolution of neutropenia and clinical symptoms. All others: continue at least 14 days after last positive culture. Concomitant rifampin: 70mg/m2 once daily. Concomitant other CYP inducers (eg, nevirapine, efavirenz, carbamazepine, dexamethasone, phenytoin): consider 70mg/m2 once daily.

  • Esophageal candidiasis: in adults is 200 mg on the first day, followed by 100 mg to 400 mg once daily.
  • The recommended dose of fluconazole for the treatment of systemic candida infections in adults is up to 400 mg once daily.
  • UTIs and peritonitis:¿¿ in adults is 50 mg to 200 mg once daily.
  • Cryptococcal meningitis:¿¿ in adults is 400 mg on the first day, followed by 200 mg to 400 mg once daily.
  • The recommended dose of fluconazole to prevent infection in adults undergoing bone marrow transplant is 400 mg once daily.
  • Oropharyngeal candidiasis: in children is 6 mg/kg on the first day, followed by 3 mg/kg once daily.
  • Esophageal candidiasis: in children is 6 mg/kg on the first day, followed by 3 mg/kg to 12 mg/kg once daily.
  • Candida infections: in children is 6-12 mg/kg once daily.
  • Cryptococcal meningitis: in children is 12 mg/kg on the first day, followed by 6 mg/kg to 12 mg/kg once daily.

Indications:
Fluconazole tablets 100mg are used to treat a variety of superficial and systemic fungal infections in various parts of the body. Fungal infections that Fluconazole tablets are used for include
  • Candidiasis (thrush) of the mucus membranes such as in the mouth and also of the skin.
  • Systemic candidiasis, like candidaemia, which is can be more serious particularly if the immune system is compromised as in AIDS, transplant or cancer patients.
  • Tinea (ringworm) of the body (tinea pedis), foot (tinea corporis) and groin (tinea cruris or jock itch).
  • Cryptococcal meningitis.
  • Fluconazole tablets 50mg/100mg are also used to prevent fungal infections like cryptococcal disease in immunocompromised patients and to prevent recurrent vaginal candidiasis.

Dosage
  • You should take your Fluconazole tablets with a glass of water with or without food usually once a day for several days or months, depending on what you are being treated for, the severity of your infection and your doctor's recommendations.  You should continue to take your Fluconazole 50 tablets for as long as recommended by your doctor and always finish your course of tablets even if you feel better, if not, the infection may not clear up completely.

  • Itraconazole capsules are indicated for the treatment of-

  • Skin infections 2 capsules (200mg itraconazole) once a day 7 days or 1 capsule (100mg itraconazole) once a day 15 days.
  • If there is a thin layer of callus on the palms of the hands or soles of the feet, it may be necessary to take 2 capsules (200mg itraconazole) twice a day for 7 days or 1 capsule (100mg itraconazole) once a day for one month.

  • IV- Loading dose 6 mg/kg 12 hourly for the 1st 24 hours. Maintenance 4 mg/kg 12 hourly.
